Permission to Publish Student Work on the Web

Dear Parents/Legal Guardians,

For the purpose of sharing with the community the exemplary work of MMSD students, the Madison Metropolitan School District publishes student work on the district Web site. We are asking permission for your child's work to be published for a reasonable period of time. If you give your permission, please sign below and return to your child's teacher.

I hereby release, hold harmless, indemnify, and defend the Madison Metropolitan School District, and its Board, officers, agents, directors, employees, and volunteers from any and all liability and legal or equitable claims of any kind related to such work being published on the district Web site. I understand that student work will be identified by first name, or first name and first initial of the last name.
